Whether vitamin D, omega-3, magnesium, or multivitamin supplements: for many people over 45, the topic has long been part of everyday life. The shelves are full, the promises are big, and online it often sounds as if fatigue, susceptibility to infections, muscle complaints, or concentration problems can almost always be solved with capsules and drops. But this is exactly where a sober look pays off. Using supplements sensibly does not mean taking as much as possible, but realistically assessing your own needs.

The basic rule is: supplements are not a substitute for a balanced diet, exercise, sleep, and medical clarification. However, they can be helpful in certain situations, for example with a proven deficiency, increased requirements, restricted nutrient absorption, or special dietary patterns. What matters is not the product, but the context.

Especially in the second half of life, the topic often becomes more important. The body changes, chronic diseases become more common, medications are added, and sometimes the diet becomes more one-sided than you think. At the same time, the desire to actively support your own health increases. That is understandable. All the more important, then, is to distinguish between sensible supplementation and unnecessary self-medication.

What supplements can actually do

Supplements provide vitamins, minerals, fatty acids, or other substances in concentrated form. Their purpose is not to treat an illness in the strict sense, but to supplement the diet. That sounds simple, but is often misunderstood in everyday life.

Anyone who eats a persistently unbalanced diet, is under a lot of stress, or takes certain medications often hopes for a quick fix. In some cases, that is justified. In other cases, however, symptoms are not due to a nutrient deficiency, but, for example, due to lack of sleep, a thyroid disorder, iron deficiency, inflammation, depression, diabetes, or side effects of medications. Then random self-medication tends to delay meaningful clarification.

In addition: not everything that feels like a deficiency actually is one. Many symptoms are non-specific. Exhaustion, muscle pain, or concentration problems can be linked to vitamins or minerals, but often have several possible causes. That is precisely why clear questions are more important than impulse purchases.

When supplements can make sense

There are clear situations in which supplements can make medical or nutritional sense. This includes not only laboratory findings, but also living circumstances and individual risks.

Typical reasons for targeted supplementation

  • proven deficiency in blood values or other examinations
  • increased requirements, for example in certain phases of life
  • restricted absorption in the intestine, for instance with chronic intestinal diseases

Especially from midlife onwards, several factors often play a role at the same time: less time spent outdoors, changed eating habits, chronic illnesses, gastrointestinal problems, or medications taken long-term. That is why the question is not only what is theoretically healthy, but what actually reaches you in your everyday life.

Special life situations can also play a role. Anyone who eats less well after an operation, is recovering from a longer illness, or has unintentionally lost weight should not underestimate their nutrient status. Targeted supplementation can make sense here—but as part of an overall assessment, not as a blanket solution.

Vitamin D: often discussed, not always automatically necessary

Vitamin D is probably the best-known topic around supplementation. The body produces it in the skin with the help of sunlight. In addition, it can be obtained to a lesser extent through foods such as fatty fish, eggs, or fortified products. In our latitudes, the body’s own production can drop significantly, especially during months with less sunshine.

Vitamin D is important for bone metabolism—i.e., for the absorption and utilization of calcium. It also plays a role in muscles and the immune system. A pronounced deficiency can show up, among other things, as muscle weakness, bone pain, or an increased risk of falls. However, non-specific symptoms such as tiredness or exhaustion on their own are not proof of a deficiency.

Who Vitamin D may be more relevant for

  • people who spend little time outdoors
  • older people with reduced skin synthesis
  • people with darker skin in low-sun regions
  • people with osteoporosis or an increased fracture risk
  • people with malabsorption in the intestine

Whether supplementation makes sense depends on the individual situation. If the risk is higher, a medical assessment or laboratory measurement may be appropriate. Very high doses taken on one’s own initiative are not a good idea, because Vitamin D can also be overdosed. Possible consequences include an elevated calcium level, nausea, kidney problems, or cardiac arrhythmias.

Omega-3 fatty acids are polyunsaturated fatty acids. EPA and DHA are particularly well known and are found mainly in fatty sea fish. They are components of cell membranes and play a role, among other things, in the cardiovascular system, the brain, and inflammatory processes.

Many people eat too little fish and therefore wonder whether Omega-3 capsules are generally useful. The answer is nuanced: anyone who regularly eats fatty fish once or twice a week often has no compelling need for supplements. Anyone who does not eat fish at all may benefit more from supplementation depending on their diet and health situation. For vegetarian or vegan diets, algae-based products are an option, as they provide DHA and in some cases EPA.

Important here as well: Omega-3 is not a general miracle cure. It replaces neither blood pressure monitoring nor exercise, smoking cessation, or a balanced diet. In addition, higher doses can affect bleeding risk in some people, especially if blood-thinning medications are already being taken. Therefore, if you have pre-existing conditions or take long-term medication, intake should be discussed with a physician.

In everyday life, the simplest question is often the most important: what does the diet actually look like? If you rarely eat fish but regularly use nuts, seeds, and plant-based oils, you do take in other omega-3 precursors, but not automatically enough EPA and DHA. Supplementation can then make sense, but it does not have to look the same for everyone.

Vitamin B12: particularly important with a vegan diet

Vitamin B12 is important for blood formation, cell division, and the nervous system. It is found almost exclusively in animal-based foods. That is why supplementation is generally necessary with a vegan diet. With a vegetarian diet, it depends on the selection and quantity of the foods consumed.

A deficiency often develops gradually. Possible signs include fatigue, concentration problems, tingling in hands or feet, unsteady gait, or changes in blood values. Especially in older people, a vitamin B12 deficiency is sometimes recognized late because the symptoms start out nonspecific.

Why vitamin B12 can be lacking even without a vegan diet

It is not only intake that matters. For vitamin B12 to be absorbed, the body needs, among other things, an intact stomach lining and enough so-called intrinsic factor, an endogenous transport protein. Gastritis, surgery on the gastrointestinal tract, or certain medications can impair absorption. These include, among others, some acid blockers and the diabetes medication metformin.

That is why dietary supplementation can make sense if risk factors are present or laboratory values suggest a deficiency.

Folic acid, iodine, and other nutrients: often situation-dependent

In addition to the well-known classics, there are other nutrients for which supplementation can be useful in individual cases. This includes, for example, folic acid, a B vitamin that is important for cell division and blood formation. A deficiency is not always easy to detect and can occur with an unbalanced diet, alcohol abuse, or certain illnesses.

Iodine also deserves attention. It is needed for the production of thyroid hormones. If you use little sea fish, dairy products, or iodized salt, you may take in too little. At the same time, iodine is not unproblematic for everyone. If you have known thyroid diseases, additional intake should be discussed with a physician.

Calcium, selenium, or certain trace elements can also become relevant, for example with a very restricted diet or on medical advice. The key point remains here as well: not every theoretically possible deficiency automatically justifies a supplement.

Magnesium, calcium, iron, and zinc: popular, but not universally advisable

With minerals, many people take them as a precaution. But here, too, the rule applies: more does not automatically help more.

Magnesium

Magnesium is often taken for muscle cramps, stress, or sleep problems. However, a genuine magnesium deficiency is rarer than assumed. Muscle cramps can have many causes, such as dehydration, nerve irritation, circulatory disorders, or medications. If a deficiency is confirmed or there is increased need, magnesium can be useful. Excessive amounts, however, often lead to diarrhea.

Calcium

Calcium is important for bones, muscles, and nerves. Many people immediately think of calcium tablets when it comes to osteoporosis. But it makes sense to look at the diet first. Dairy products, calcium-rich mineral water, some types of vegetables, and certain plant-based products can already contribute a lot. Additional intake should be considered especially when intake is clearly too low or there is an increased risk of bone loss.

Iron

Iron should not be taken without a specific reason. Fatigue alone does not automatically mean iron deficiency. Before taking it, blood values are important—especially hemoglobin and ferritin, i.e., iron stores. An iron deficiency should be taken seriously because it can indicate blood loss, absorption problems, or other diseases.

Zinc

Zinc is involved in many metabolic processes, including the immune system and wound healing. In the short term, it may be useful in individual cases; however, permanently high doses are not harmless. They can, for example, interfere with copper absorption.

Who has a higher risk of deficiency states

A blanket vitamin check for everyone is not always necessary. However, there are groups where a closer look can make sense.

  • People over 65 with loss of appetite or unintentional weight loss
  • People with chronic gastrointestinal diseases
  • People after bariatric surgery or major bowel surgery
  • People following a vegan diet
  • People with osteoporosis or frequent falls
  • Patients on multiple long-term medications
  • People with alcohol problems or a highly unbalanced diet

Especially at older age, not only energy intake declines, but often protein and micronutrient intake as well. At the same time, diseases and medications increase. This can promote deficiency states without immediately causing clear symptoms.

Chewing problems, ill-fitting dentures, a reduced sense of smell and taste, or loneliness can also worsen nutrition. These factors are easily overlooked in everyday life, but for nutrient supply they are often more important than any supplement.

Which symptoms can indicate a deficiency

Deficiency symptoms are often non-specific. Fatigue, pallor, muscle weakness, concentration problems, hair loss, susceptibility to infections, or tingling can fit with a nutrient deficiency, but they have many other possible causes. That is exactly why it is problematic to treat symptoms prematurely with dietary supplements.

It is different when there are known risk factors or abnormal findings. Then targeted diagnostics can help. Which blood values make sense depends on the question. Often relevant are, for example, vitamin B12, ferritin, vitamin D, folic acid, calcium, or, if needed, additional laboratory values.

Warning signs that should be medically clarified

Interactions and dosage: why more is not automatically better

A common mistake is taking several products in parallel without checking the total dosage. For example, anyone using a multivitamin product, additional magnesium, and a special immune product may be taking some substances in double or triple amounts.

This is not only unnecessary, it can also become problematic. Minerals can interfere with each other’s absorption. Iron, for example, is absorbed differently than calcium, and some products should not be taken at the same time as certain medications. Thyroid medications, osteoporosis drugs, or antibiotics can also be affected.

In addition, the following applies: Over-the-counter does not automatically mean harmless. What matters are dosage, duration of use, pre-existing conditions, and the question of whether there is any need at all.

Risks of supplements: “natural” is not automatically harmless either

Many products seem harmless because they are available over the counter. Nevertheless, they can cause side effects, interactions, and overdosing. Fat-soluble vitamins such as vitamin D or vitamin A are stored in the body and can accumulate. Minerals can affect the absorption of other nutrients or trigger gastrointestinal complaints.

What you should pay particular attention to

  • do not take high-dose products without a clear reason
  • follow the package information and recommended daily amounts
  • check multiple products for duplicates, for example in multivitamin products
  • ask your doctor if you take blood thinners, thyroid medications, or have kidney disease
  • take symptoms that occur while taking them seriously

Herbal products or products marketed as natural can also be problematic. It becomes particularly critical when many remedies are taken at the same time and you lose track.

What you can do yourself before you turn to supplements

In many cases, it is worth taking an honest look at everyday life first. Anyone who skips meals, eats hardly any fresh foods, moves little, and sleeps poorly will rarely benefit noticeably from capsules alone.

Sensible first steps

  • review your diet: enough protein, vegetables, legumes, whole grains, nuts, and high-quality fats?
  • eat regularly instead of just snacking throughout the day
  • if you avoid fish, consider alternatives for omega-3
  • if possible in everyday life, get regular daylight exposure
  • go through your medication list with your doctor
  • do not attribute symptoms solely to vitamins

Often, that already achieves a lot. Supplements are then used in a more targeted, safer, and more realistic way.

A food diary over a few days can also be practical. It often shows more clearly than gut feeling whether certain food groups are consistently falling short. If you recognize patterns, you can start with the basics first and then better assess whether a product is needed at all.

When you should seek medical advice

A visit to the doctor is particularly advisable if symptoms persist for a longer period, worsen, or if several symptoms occur together. Medical advice should also be sought before longer-term use or higher dosing, especially if you have pre-existing conditions.

This applies, for example, in cases of severe fatigue, unintended weight loss, numbness, persistent muscle pain, frequent falls, chronic bowel complaints, or if you are already taking medication. Then it is not only about whether a supplement might help, but also about not overlooking serious underlying causes.

Consultation is also particularly important in cases of kidney disease, liver disease, osteoporosis treatment, thyroid disorders, and blood-thinning medication. In these situations, even over-the-counter products can have more impact than many assume.

Conclusion: targeted use instead of taking everything “just in case”

Using dietary supplements sensibly means understanding the difference between a potential need and mere hope. Vitamin D, omega-3, vitamin B12, or individual minerals can be helpful in certain situations. For healthy people with a balanced diet, however, taking them across the board is often unnecessary.

You benefit most from a sober strategy: take symptoms seriously, check risk factors, improve your diet, determine laboratory values if needed, and use supplements only in a targeted way. This way, dietary supplementation is neither demonized nor overestimated, but used where it can actually make a contribution.

The most important point, therefore, is not which product is currently particularly popular, but which question you actually want to answer. Is it about a suspected deficiency, a specific diet, an illness, or general preventive healthcare? The clearer this question is, the more likely you are to reach a well-founded decision.
